Notice of Privacy Practices

This notice describes how medical information about your child may be used and shared, and how you can get access to this information. Please read it carefully.

Anita Sabeti, MD, doing business as Beverly Hills Pediatrician, is required by law to keep your child's health information private. We are also required to give you this notice explaining our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How We May Use and Share Your Child's Health Information

For Treatment

We may use and share your child's health information with doctors, nurses, and other staff members who are involved in providing care. For example, if your child sees a specialist, we may share test results or records so the specialist can provide the right treatment.

For Payment

We may use and share health information to bill and collect payment for services, including preparing a superbill you can submit to your insurance company for possible reimbursement.

For Health Care Operations

We may use and share health information to run our practice, improve care, and make sure all patients receive quality service. This can include staff training, quality reviews, and business planning.

Appointment Reminders and Health Information

We may contact you by phone, text message, email, or through our patient portal to remind you of appointments, or to give you information about treatment options or other health related services and benefits that may be of interest to you.

As Required by Law

We will share health information when required to do so by federal, state, or local law. This includes situations such as suspected child abuse or neglect, court orders, and public health reporting requirements, for example certain communicable diseases or immunizations.

To Avoid a Serious Threat to Health or Safety

We may share health information when necessary to prevent a serious threat to your child's health and safety, or the health and safety of the public or another person.

Your Rights

You have the following rights regarding your child's health information. To exercise any of these rights, please contact our office using the information at the bottom of this page.

  • Get a copy of your child's paper or electronic medical record. You can ask to see or receive a copy of your child's medical record and other health information we have. We will provide a copy or summary within a reasonable time, usually within 30 days, and may charge a reasonable fee.
  • Ask us to correct your child's medical record. You can ask us to correct health information you believe is incorrect or incomplete. We may decline your request, and we will explain our reason in writing within 60 days.
  • Request confidential communications. You can ask us to contact you in a specific way, such as a home phone number rather than a work number, or to send mail to a different address. We will agree to all reasonable requests.
  • Ask us to limit what we use or share. You can ask us not to use or share certain health information for treatment, payment, or operations. We are not required to agree, but if we do agree, we will honor that request unless the information is needed for emergency treatment.
  • Get a list of those with whom we have shared information. You can request a list, called an accounting of disclosures, of the times we shared health information for reasons other than treatment, payment, or health care operations, going back up to six years.
  • Get a paper copy of this notice. You have the right to a paper copy of this notice at any time, even if you agreed to receive it electronically.
  • Choose someone to act on your behalf. If you have given someone medical power of attorney or if someone is your child's legal guardian, that person can exercise your child's rights and make choices about their health information.
  • File a complaint if you feel your rights are violated. You can file a complaint with our office or with the U.S. Department of Health and Human Services Office for Civil Rights. Filing a complaint will not change the care your child receives from us, and we will not retaliate against you for filing one.

Our Responsibilities

  • We are required by law to maintain the privacy and security of your child's protected health information.
  • We must notify you promptly if a breach occurs that may have compromised the privacy or security of your child's information.
  • We must follow the duties and privacy practices described in this notice, and give you a copy of it.
  • We will not use or share your child's information other than as described here, unless you give us written permission to do so. If you give us permission, you may withdraw it in writing at any time.

Changes to This Notice

We may change the terms of this notice at any time. Any new notice will apply to health information we already have, as well as any information we receive in the future. We will post the current notice on our website, and a printed copy will be available in our office upon request.
